As a patient of Life Changing Wellness and Treatment Center, you have important rights and responsibilities. Understanding these rights helps ensure you receive the best possible care.
Your Rights as a Patient
In accordance with Arizona law (A.R.S. Title 36) and AHCCCS standards, you have the right to:
- Receive care without discrimination based on race, color, religion, sex, national origin, age, disability, sexual orientation, or gender identity
- Be treated with dignity, respect, and courtesy at all times
- Privacy and confidentiality of your behavioral health records in accordance with HIPAA and applicable Arizona law
- Receive a clear explanation of your diagnosis, treatment options, and the risks and benefits of proposed treatments in language you understand
- Participate actively in the development of your Individualized Service Plan (ISP) and treatment goals
- Refuse treatment, to the extent permitted by law, and to be informed of the medical consequences of refusal
- Access your own health records and request amendments as permitted by law
- Receive information about the services available to you and how to access them
- Be informed of any changes to your treatment plan
- File a complaint or grievance without fear of retaliation or discrimination
- Receive a written copy of your patient rights upon request
- Know the names and professional status of the staff providing your care
- Receive services in the least restrictive, most integrated setting appropriate to your needs
- Continuity of care and timely discharge planning when services are ending
Your Responsibilities as a Patient
To help us provide you with the best possible care, we ask that you:
- Arrive on time for scheduled appointments and notify us as soon as possible if you need to cancel or reschedule
- Provide accurate and complete information about your health history, medications, and any changes in your condition
- Follow the treatment plan you have agreed to and communicate openly with your care team if you have concerns
- Treat staff, other patients, and visitors with respect
- Refrain from bringing alcohol, illegal substances, or weapons onto facility premises
- Notify us of any changes to your insurance, contact information, or emergency contacts
- Ask questions when you do not understand your diagnosis, treatment, or medications
- Accept responsibility for the consequences if you refuse treatment or do not follow your care plan
Grievance and Appeals Process
If you have a complaint about your care or believe your rights have been violated, you have the right to file a grievance. You will not face retaliation for doing so.
To file a grievance:
- Speak directly with your therapist, case manager, or the Clinical Director
- Submit a written complaint to our office at the address below
- Contact AHCCCS if your grievance is not resolved to your satisfaction

Advance Directives
You have the right to prepare an Advance Directive (such as a healthcare power of attorney or mental health care directive) to express your wishes about future treatment decisions. Ask your case manager or therapist for more information.
