Getting Started
No referral is needed. You can contact us directly by phone, email, or through our website to schedule an intake appointment. If you are an AHCCCS member, you may call your health plan to request an authorization, but our team can also help guide you through that process.
Please bring a valid photo ID, your insurance card (including your AHCCCS ID if applicable), any current medications or a medication list, emergency contact information, and any previous behavioral health records if available.
Your initial intake appointment typically takes 60 to 90 minutes. During this time, our team will complete a comprehensive behavioral health assessment, review your history, discuss your goals, and begin developing your Individualized Service Plan (ISP).
We recommend scheduling an appointment in advance to ensure a clinician is available for your intake. However, if you are in crisis or have an urgent need, please call us at (602) 319-5155 and we will do our best to see you as soon as possible.
Yes, we offer telehealth services for eligible patients. Telehealth may be appropriate for follow-up therapy sessions, case management check-ins, and certain other services. Ask our team whether telehealth is right for your situation.
Insurance & Payment
Yes. We are a contracted AHCCCS provider and accept most AHCCCS managed care plans including Mercy Care, Banner University Family Care, UnitedHealthcare Community Plan, and Arizona Complete Health.
We offer a sliding scale fee structure based on income for uninsured or underinsured patients. We believe cost should never be a barrier to getting behavioral health care. Contact us to discuss your options.
Call us at (602) 319-5155 and our intake team will verify your benefits at no cost before your first appointment. We will explain what is covered and what, if anything, you may owe.
We ask that you give us at least 24 hours notice if you need to cancel or reschedule. Repeated no-shows without notice may affect your continued enrollment in services.
Treatment & Services
Outpatient (OP) typically involves one to two sessions per week and is appropriate for patients who are stable and need ongoing support. Intensive Outpatient (IOP) is a more structured program involving multiple sessions per week, designed for patients who need more support than standard outpatient but do not require inpatient care.
Treatment length depends on your individual needs and goals. Some patients reach their goals in a few months; others benefit from longer-term services. Your care team will regularly review your progress and adjust your plan accordingly.
A BHT is a trained staff member who works alongside licensed clinicians to provide direct support to patients. They may assist with group sessions, skill-building activities, transportation coordination, and daily living support.
An ISP is a personalized treatment plan developed collaboratively by you and your care team. It outlines your goals, the services you will receive, and the steps you and your team will take to support your recovery.
We offer psychiatric evaluation and medication management services. Your prescribing provider will work closely with your therapist and case manager to ensure your medications are supporting your overall treatment goals.
Privacy & Rights
Yes. Your behavioral health information is protected under HIPAA and Arizona law. We will not share your information without your written consent except in limited circumstances required by law, such as mandatory reporting of abuse or imminent safety concerns.
Yes. You have the right to request access to your behavioral health records. Submit a written request to our office and we will respond within 30 days as required by law.
You can speak directly with your therapist, case manager, or our Clinical Director at any time. You may also file a formal grievance in writing. You will never face retaliation for raising a concern. You may also contact AHCCCS at 1-800-654-8713.
